Osteoporosis Prevention After 40: The India-Specific Action Plan

A female doctor with a stethoscope studying an X-ray at her desk beside a laptop.

Women over 50 and men over 65 should have a DEXA scan to check their bone density.

Osteoporosis is called the "silent disease" — and in India, it earns that name more fully than almost anywhere. Bone density begins declining in the mid-30s. By the time a vertebral compression fracture or a hip fracture brings it to clinical attention, 20 to 30 years of preventable bone loss may already have occurred.

India has one of the highest rates of osteoporosis-related fractures in the world — partly due to population size, partly due to near-universal Vitamin D deficiency, and partly because bone health is almost never discussed in Indian preventive healthcare until a fracture happens.

The hip fracture that happens at 72 is preventable by action taken at 42. This guide describes what those actions are, specifically for the Indian context.

Who Is at Highest Risk in India

Women after menopause: Oestrogen protects bone. Menopause — which in Indian women tends to occur slightly earlier (average 46 to 47 years) than in Western women (51 years) — triggers a period of accelerated bone loss. Without intervention, a woman can lose 1 to 3 percent of her bone mass per year in the first 5 years after menopause.

All Indians for Vitamin D deficiency: India sits between 8 and 37 degrees latitude — theoretically sufficient sunlight for Vitamin D synthesis. In practice, dark skin (more melanin = lower Vitamin D synthesis per unit of sunlight), indoor working lives, full-body clothing in many communities, and air pollution blocking UV radiation mean that Vitamin D deficiency is the norm rather than the exception across India, regardless of latitude. Studies consistently show 70 to 90 percent of urban Indians have Vitamin D deficiency (below 20 ng/mL) or insufficiency (20 to 30 ng/mL).

Vitamin D is essential for calcium absorption. Without adequate Vitamin D, even high calcium intake cannot be properly absorbed and utilised for bone mineralisation.

Men over 65: Osteoporosis is consistently underdiagnosed in Indian men, who are less likely to be screened and less likely to associate themselves with a "women's condition." Men lose bone more slowly than post-menopausal women but are equally susceptible to the consequences — particularly hip fractures.

Long-term steroid users: Corticosteroids (prescribed widely in India for asthma, inflammatory conditions, and skin disease) are one of the most potent causes of secondary osteoporosis. Any patient on oral prednisolone for more than 3 months should be assessed for bone density and managed with bone protection.

Patients with rheumatoid arthritis, hyperthyroidism, chronic kidney disease, and inflammatory bowel disease: All of these conditions accelerate bone loss through specific mechanisms.

The Investigation You Need First: DEXA Scan

A DEXA (dual-energy X-ray absorptiometry) scan measures bone mineral density (BMD) at the lumbar spine and hip and produces a T-score — the number of standard deviations the patient's BMD falls below the average peak bone mass of a young adult.

T-score interpretation:

  • Above -1.0: Normal bone density
  • Between -1.0 and -2.5: Osteopenia (low bone density, increased fracture risk)
  • Below -2.5: Osteoporosis (significantly reduced bone density, high fracture risk)

Who should get a DEXA scan in India:

  • All women over 50, particularly post-menopausal
  • Women under 50 with risk factors (family history, early menopause, low BMI, prolonged steroid use)
  • All men over 65
  • Men of any age on long-term steroids or with identified risk factors
  • Any patient who has had a fragility fracture (a fracture from a minor fall or trivial trauma)

DEXA scanning is available in Noida and Greater Noida at multiple radiology centres. It is inexpensive, takes 10 to 15 minutes, and involves minimal radiation — less than a standard chest X-ray.

The Lifestyle Foundation: What Works

Calcium — The Building Block

Bone is primarily calcium phosphate. Adults need 1,000 to 1,200 mg of calcium per day — and most Indian adults consume significantly less than this through diet alone.

India-specific high-calcium foods:

  • Ragi (finger millet): 344 mg calcium per 100g — the richest plant-based calcium source commonly available in India. Two ragi rotis daily provides 200 to 250 mg calcium.
  • Sesame seeds (til): A tablespoon provides approximately 90 mg calcium. Used in chutney, laddoos, and cooking.
  • Dairy: One glass of milk (250 ml) provides approximately 300 mg calcium. A katori of curd provides 150 to 200 mg.
  • Spinach (palak): Approximately 100 mg per 100g — but contains oxalates that reduce absorption. Still useful.
  • Amla (Indian gooseberry): High Vitamin C content aids calcium-related processes.

The supplement question: If dietary calcium is consistently below 800 mg/day (very common in urban Indian adults who are lactose intolerant or have low dairy intake), calcium carbonate or calcium citrate supplements are appropriate. Calcium is best absorbed in doses of 500 mg or less — splitting supplements (morning and evening) is more effective than a single 1,000 mg dose.

Vitamin D — Non-Negotiable in India

Virtually every Indian adult who is not supplementing Vitamin D has suboptimal levels. This is not hyperbole — it is what the studies show.

Supplement recommendation: 1,000 to 2,000 IU of Vitamin D3 daily for maintenance in adults who are not deficient. For documented deficiency (level below 20 ng/mL), loading doses (60,000 IU weekly for 8 to 12 weeks) followed by maintenance doses are typically prescribed.

Sunlight: 15 to 20 minutes of direct sunlight on the face, arms, and hands (not through glass) between 10am and 3pm provides meaningful Vitamin D synthesis in lighter-skinned individuals. For darker skin or in highly polluted urban environments, sunlight alone is typically insufficient to maintain adequate levels.

The Vitamin D-calcium interaction: Vitamin D enables calcium absorption in the gut. Without adequate Vitamin D, calcium supplements and dietary calcium are poorly absorbed. Both need to be adequate simultaneously.

Weight-Bearing Exercise — The Bone Loading Signal

Bone remodelling is stimulated by mechanical stress — the bone responds to load by building denser structure. Weight-bearing exercise (walking, jogging, stair climbing, dancing, yoga standing poses) is more stimulating for bone than non-weight-bearing exercise (swimming, cycling).

Resistance training is the most potent exercise stimulus for bone density — particularly in the spine, hip, and wrist, the three sites most vulnerable to osteoporotic fracture. Lifting weights generates direct compressive and tensile loads on these bones, triggering bone formation.

For Indian adults over 40, a programme combining:

  • 30 minutes of brisk walking, 5 days weekly
  • Two sessions of resistance training weekly (bodyweight exercises, light weights, resistance bands)

…is both achievable and clinically meaningful for bone density maintenance.

Medical Treatment: When Lifestyle Is Not Enough

When DEXA scanning shows osteopenia or osteoporosis, and particularly when a fragility fracture has already occurred, lifestyle measures alone are insufficient. Medical bone protection is needed.

Bisphosphonates — The First-Line Treatment

Bisphosphonates (alendronate, risedronate, zoledronic acid) reduce the activity of osteoclasts — the bone cells responsible for bone breakdown. This shifts the balance toward bone formation and measurably increases bone density at the spine and hip.

Alendronate (70 mg weekly tablet) is the most commonly used oral bisphosphonate. It must be taken with a full glass of water, in the upright position, 30 minutes before food — to avoid oesophageal irritation. Patients who cannot tolerate oral bisphosphonates have the option of intravenous zoledronic acid (annual infusion).

Bisphosphonates reduce vertebral fracture risk by approximately 50 percent and hip fracture risk by 30 to 40 percent in clinical trials — meaningful reductions that translate to enormous quality-of-life differences.

Hormone Replacement Therapy (HRT)

For post-menopausal women in the early post-menopausal window (within 10 years of menopause), HRT is both the most effective bone protection and the most effective relief of menopausal symptoms. The appropriate use of HRT after menopause — when the benefits outweigh risks for the individual patient — is a discussion with a gynaecologist in the context of the complete medical history.

Calcium and Vitamin D Supplementation

Prescribed formally (rather than as an afterthought), adequate calcium (500 to 600 mg twice daily if dietary intake is low) and Vitamin D (1,000 to 2,000 IU daily) are essential co-prescriptions with any bone-protective medication.

The Fracture That Must Not Happen: Why Hip Fractures Are Different

Hip fractures in elderly patients are not "just a broken bone." In India, hip fracture in patients over 70 carries significant mortality — some studies show 20 to 30 percent one-year mortality after hip fracture in elderly Indian patients, a figure driven by immobility-related complications (pneumonia, deep vein thrombosis, pressure sores), deconditioning, and the psychological impact of sudden functional loss.

Every preventive measure taken in the 40s and 50s — calcium, Vitamin D, weight-bearing exercise, DEXA scanning, treating identified osteopenia — is, at its core, hip fracture prevention decades in the future.

The falls prevention component of osteoporosis management deserves specific mention in the Indian context: good lighting at night (most hip fractures happen on the way to the toilet at 2am), removal of floor-level obstacles (most Indian homes have many), and handrails in bathrooms reduce the risk of the fall that turns inadequate bone into a fracture.

Frequently Asked Questions

I am 45, female, and had an early menopause at 43. Should I start treatment now? Early menopause accelerates bone loss — the oestrogen protection is lost earlier. A DEXA scan now is appropriate to establish your baseline. If already in the osteopenic range, discussion with a specialist about bone protection (and possibly HRT if no contraindications) is warranted earlier than the standard screening age.

Is calcium supplementation dangerous for the heart? Some earlier studies raised concern that calcium supplements increase cardiovascular risk. More recent and more rigorous analyses suggest that calcium supplements within normal dose ranges (500 to 1,000 mg daily, in addition to dietary calcium) do not significantly increase cardiovascular risk when Vitamin D is adequate. Exceeding 1,500 mg of supplemental calcium daily is not recommended.

Can I get my Vitamin D from diet alone? Very few foods contain significant Vitamin D — fatty fish (salmon, mackerel, sardines), egg yolks, and fortified foods (some milks and cereals). For most Indian adults, dietary Vitamin D intake alone is inadequate to maintain optimal levels. Supplementation is practical and inexpensive.
