गर्दन में दर्द क्यों होता है? कारण, लक्षण और सही उपचार की पूरी जानकारी

गर्दन का दर्द (neck pain) आज urban India में सबसे common orthopedic complaints में से एक बन गया है। Mobile phones, laptops, और desk jobs ने इसे almost epidemic बना दिया है। मेरे clinic में गर्दन दर्द की complaints पिछले 5 सालों में almost double हो गई हैं, और ज्यादातर patients 25-45 age group के हैं।

गर्दन दर्द के मुख्य कारण

1. Poor Posture, सबसे बड़ा Villain

आपके सिर का weight करीब 5 kg है। जब सिर spine के ऊपर balanced हो — neck muscles पर load manageable रहता है। लेकिन जब आप phone देखते हो या laptop पर झुकते हो, तो हर inch forward tilt = neck पर double load। Typical "phone posture" में neck muscles 15-20 kg effective weight support कर रही होती हैं, hours तक, daily, years तक।

Result: Muscles spasm, tendons inflame, disc degeneration accelerate, और eventually chronic neck pain।

2. cervical spondylosis (Degenerative changes)

Age-related wear, cervical spine की discs और joints में degenerative changes। 40+ age में ये almost universal है — X-ray में दिखता है, लेकिन हर किसी को symptoms नहीं होते। Bone spurs (osteophytes) form होते हैं, disc height कम होती है, joint surfaces rough होती हैं।

लक्षण: Neck stiffness (especially morning), dull ache, movement में restriction, कभी-कभी grinding sound।

3. muscle strain/Spasm

Acute overuse, wrong sleeping position, sudden movement, stress, ये सब neck muscles में strain और spasm cause कर सकते हैं। ये सबसे common acute neck pain है, usually self-limiting (3-7 days में better)।

4. cervical disc herniation

Disc का soft center बाहर निकलता है और nerve पर pressure डालता है — arm में radiating pain, numbness, या weakness produce होता है (cervical radiculopathy)।

Red flag symptoms: Arm में shooting pain, specific fingers में numbness, grip weakness।

5. cervical stenosis

Spinal canal narrow हो जाता है, spinal cord compress होती है। Serious condition।

Symptoms: Hands में clumsiness, walking balance problems, legs में stiffness, bladder issues। ये urgent evaluation चाहती है।

6. Stress और Anxiety

Mental stress directly neck और shoulder muscles tighten करता है। "Carrying stress in your shoulders", ये literally true है। Chronic stress = chronic muscle tension = chronic neck pain।

7. अन्य कारण

  • Whiplash (car accident — sudden head jerk)
  • Fibromyalgia (widespread pain condition)
  • Infections (meningitis, fever + severe neck stiffness, EMERGENCY)
  • Tumors (rare, but persistent night pain warrants evaluation)

लक्षण Types

Mechanical Neck Pain (सबसे common)

  • Dull, aching pain
  • Stiffness — especially morning या long sitting बाद
  • Pain specific movements से worse, turning head, looking up
  • Muscle tenderness, shoulders और upper back में
  • Headache — back of head (tension headache)

Radiculopathy (Nerve involvement)

  • Sharp, shooting pain arm में, shoulder से fingers तक
  • Numbness/tingling specific fingers में (which fingers = which nerve level)
  • Arm weakness, grip कम, objects drop
  • Pain coughing या sneezing से worse

Myelopathy (Spinal cord compression) — SERIOUS

  • Hands clumsy, buttons lगाने में difficulty, writing बदल गई
  • Walking unsteady, balance problems
  • Legs stiff
  • Bladder control issues
  • तुरंत doctor दिखाएं

Diagnosis

Clinical examination: Range of motion, neurological examination (reflexes, sensation, strength), Spurling test (nerve compression check), and specific palpation।

Imaging:

  • X-ray: Bone alignment, disc height, spurs, spondylosis changes
  • MRI: Disc herniation, nerve/cord compression, soft tissue detail — gold standard for nerve symptoms
  • CT scan: Bone detail, fractures, complex spondylosis
  • Nerve conduction studies: Confirm which nerve affected and damage extent

इलाज

घर पर (Mild-Moderate Pain)

Posture correction, सबसे ज़रूरी:

  • Screen eye level पर रखो — laptop stand या books use करो
  • Phone ऊपर hold करो, गर्दन झुकाकर मत देखो
  • Chair में lumbar + cervical support
  • 20-20-20 rule: हर 20 minutes, 20 seconds के लिए 20 feet दूर देखो + neck stretch

Exercises (daily 10 minutes):

  • Chin tucks: ठोड़ी अंदर लो (double chin बनाओ), 5 seconds hold, 10 reps, ये सबसे effective neck exercise है
  • Neck isometrics: हाथ से forehead, sides, और back of head पर resistance दो, neck muscles contract करो बिना head move किए। 5 seconds × 10 reps each direction
  • Shoulder blade squeezes: Shoulders पीछे squeeze करो, 5 seconds hold, 15 reps
  • Gentle rotation: Slowly left-right turn, 10 reps each
  • Side bending: Ear towards shoulder (gently), 10 reps each

Heat/Cold:

  • Acute pain (first 48 hours): Ice — 15 minutes, 3x daily
  • Chronic stiffness: Warm compress, 15-20 minutes, blood flow improve

OTC medications: Paracetamol या topical diclofenac gel, short term।

Medical treatment

Physiotherapy: Structured program — 6-8 sessions, exercises, manual therapy, ergonomic education। Most patients significantly improve।

Medications:

  • NSAIDs (short course)
  • Muscle relaxants (tizanidine, thiocolchicoside)
  • Nerve pain medicine (pregabalin, gabapentin), अगर radiculopathy है

Cervical collar: बहुत short-term use only (2-3 days maximum for acute pain)। Prolonged use muscles weaken करता है — avoid।

Injections:

  • Trigger point injections, painful muscle knots में
  • Epidural steroid injection, nerve compression pain में
  • Facet joint injection — facet arthropathy में

Surgery (Rare, <5% cases)

Surgery तभी:

  • Progressive neurological deficit (worsening weakness/numbness)
  • Myelopathy signs (cord compression)
  • Severe radiculopathy not responding to 6-12 weeks conservative treatment
  • Significant structural instability

Procedures: ACDF (anterior cervical discectomy and fusion), cervical disc replacement, posterior laminectomy।

Prevention

  • Ergonomic workspace setup, screen height, chair support, keyboard position
  • Phone habits बदलें — ऊपर hold करो, duration limit करो
  • Regular breaks, हर 30-45 minutes neck stretches
  • Pillow check, cervical pillow या proper height pillow — neck neutral position में रहे
  • Sleep position, side या back best। Pet ke bal sona worst for neck।
  • Exercise regularly, neck, shoulder, और core strengthening
  • Stress management — deep breathing, yoga, meditation, muscles relax

Doctor से कब मिलें?

  • Neck pain 2 weeks से ज्यादा
  • Arm में numbness, tingling, या weakness
  • Severe headache जो neck pain के साथ हो
  • Fever + neck stiffness (possible meningitis, EMERGENCY)
  • Walking balance problems
  • Hands clumsy हो रही हों
  • Pain night में भी disturb करे
  • Accident/injury के बाद neck pain

गर्दन का दर्द ज्यादातर lifestyle-related है — और lifestyle changes से ही ठीक होता है। Posture, exercises, और ergonomic awareness, ये तीन चीज़ें 80%+ neck pain resolve कर सकती हैं। लेकिन nerve symptoms (arm numbness, weakness) हों तो delay मत करें, early evaluation better outcomes देता है।
