बार-बार कंधे में जकड़न या दर्द क्यों होता है?

कंधे का दर्द एक ऐसी problem है जो बहुत common है लेकिन अक्सर seriously नहीं ली जाती, जब तक कि बालों में कंघी करना, कपड़े पहनना, या ऊपर रखी चीज़ उठाना impossible न हो जाए। मेरे clinic में shoulder complaints knee pain के बाद दूसरी सबसे common problem है। और इसमें एक pattern मैं बार-बार देखता हूँ: patients months तक "adjust" करते रहते हैं और तब आते हैं जब shoulder practically frozen हो चुका होता है।

कंधा इतना Vulnerable क्यों है?

कंधा body का सबसे mobile joint है, ball-and-socket design जो 360 degrees movement allow करती है। लेकिन ये mobility एक trade-off के साथ आती है: stability कम होती है। कंधे को muscles, tendons, और ligaments stable रखते हैं — हड्डी का support minimal है।

इसका मतलब: कंधे की muscles और tendons पर बहुत ज्यादा load पड़ता है। ये structures थकते हैं, irritate होते हैं, और damage होते हैं, खासकर age बढ़ने के साथ।

बार-बार कंधे में जकड़न/दर्द के मुख्य कारण

1. Rotator Cuff Problem, सबसे Common कारण

Rotator cuff 4 muscles का group है (supraspinatus, infraspinatus, teres minor, subscapularis) जो shoulder joint को stable रखता है और arm movements control करता है। 40+ age में इन tendons का degeneration शुरू हो जाता है।

Rotator Cuff Tendinitis: Tendons inflame हो जाते हैं — usually overuse या repetitive overhead movements से। दर्द mainly arm ऊपर उठाने में, पीछे ले जाने में, और रात को side में सोने पर।

Rotator Cuff Tear: Tendon partially या completely फट जाता है, injury से या gradual degeneration से। मैं 50+ patients में partial tears बहुत frequently देखता हूँ। Complete tear में arm raise करना very difficult हो जाता है।

कैसे पहचानें: दर्द shoulder के side में होता है (not on top), रात को worse, overhead activities में ज्यादा, और weakness feel होती है arm lift करने में।

2. frozen shoulder (Adhesive capsulitis)

ये condition shoulder capsule (joint को cover करने वाली membrane) thick और tight हो जाती है, और adhesions (scar tissue) form हो जाते हैं। Result: severe stiffness + pain। Patient अपनी arm को actively या passively (दूसरे हाथ से भी) पूरा move नहीं कर पाता।

3 Stages:

  1. Freezing (2-9 months): Pain gradually बढ़ता है, movement कम होने लगती है
  2. Frozen (4-12 months): Pain थोड़ा कम हो सकता है, लेकिन stiffness maximum
  3. Thawing (5-24 months): Gradually movement improve होती है

Who gets it: 40-60 age, women ज्यादा, diabetics में 10-20% chance (non-diabetics से 5 गुना ज्यादा), thyroid patients।

Important: Frozen shoulder self-limiting है, eventually ठीक होता है — लेकिन बिना treatment 2-3 साल लग सकते हैं। Proper treatment से timeline significantly short होती है।

3. impingement syndrome

Shoulder में arm ऊपर उठाते समय rotator cuff tendons acromion (shoulder blade का bony point) और humerus (arm bone) के बीच pinch/compress होते हैं। Repeated impingement से tendons irritate होते हैं, inflame होते हैं, और eventually tear हो सकते हैं।

लक्षण: Arm 60-120 degrees raise करने पर दर्द, इस range को "painful arc" कहते हैं। ऊपर या नीचे ठीक, बीच में problem।

4. cervical spine problems (Referred pain)

कभी-कभी shoulder pain actually neck (cervical spine) से आता है। Cervical disc bulge या nerve compression shoulder area में referred pain produce कर सकता है। ये तब suspect करता हूँ जब:

  • Shoulder examination relatively normal हो
  • Neck movement से shoulder pain trigger हो
  • Arm में numbness/tingling भी हो

5. bursitis

Subacromial bursa (shoulder में cushion) inflame हो जाता है, repetitive overhead activities, sleeping on one side, या impingement की वजह से।

6. lifestyle factors

  • Long hours phone/laptop use — forward head posture shoulder muscles पर chronic stress डालता है
  • Wrong sleeping position, एक ही side पर घंटों सोने से shoulder compress होता है
  • Lack of exercise, shoulder muscles weak होने से tendons पर ज्यादा load
  • Sudden intense activity — हफ्ते भर कुछ नहीं और Sunday को intense badminton/cricket

Diagnosis

Clinical examination

मैं specific tests करता हूँ, Neer test, Hawkins test (impingement), drop arm test, empty can test (rotator cuff), external rotation assessment (frozen shoulder)। ये tests 80-90% cases में diagnosis बता देते हैं।

Imaging

  • X-ray: Bone spurs, calcification, arthritis check। Soft tissue नहीं दिखती।
  • Ultrasound: Rotator cuff tears, bursitis, और tendinitis, in-clinic जल्दी check हो जाता है।
  • MRI: Gold standard — rotator cuff tear size, labral tears, और other soft tissue pathology clearly दिखती है। Surgery plan करने से पहले usually order करता हूँ।

Treatment

Conservative (ज्यादातर cases में)

Physiotherapy, backbone of treatment:

  • Pendulum exercises (Codman's)
  • Wall climbing exercises
  • External rotation with resistance band
  • Scapular stabilization exercises
  • Stretching, cross-body stretch, towel stretch behind back

Consistently 6-8 weeks physiotherapy करने से 70-80% shoulder problems significantly improve होती हैं।

Medications: NSAIDs (short course), muscle relaxants, topical gels।

Ice/Heat: Acute pain में ice (15 minutes, 3x/day), chronic stiffness में heat।

Corticosteroid injection: Subacromial space में injection — significant pain relief 3-5 days में। Physiotherapy शुरू करने का window देती है। Limit: 3 injections/year maximum।

Surgical (selected cases)

  • Arthroscopic subacromial decompression, impingement के लिए
  • Rotator cuff repair, significant tear जो conservative treatment से ठीक न हो
  • Manipulation under anesthesia — severe frozen shoulder जो physiotherapy से respond न कर रहा
  • Shoulder replacement, severe arthritis में (less common than knee/hip replacement)

Prevention

  • Shoulder exercises regularly करें, resistance band external rotation, wall push-ups, scapular squeezes
  • Posture ठीक रखें — shoulders back, chin tucked
  • Sleeping position, दर्द वाले side पर मत सोएं; back sleeping best है shoulder problems के लिए
  • Warm-up, किसी भी overhead activity से पहले
  • Screen height adjust करें — eye level पर, ताकि forward head posture न हो

Doctor से कब मिलें?

  • Shoulder pain 2-3 हफ्ते से ज्यादा हो
  • Arm raise करने में difficulty बढ़ रही हो
  • Night pain से नींद disturb हो रही हो
  • कंधे में stiffness progressively बढ़ रही हो
  • किसी injury के बाद arm उठाने में weakness हो
  • दोनों hands से back में reach नहीं कर पा रहे (bra strap/wallet pocket test)

कंधे की problems slowly develop होती हैं और early stage में easily treatable होती हैं। Delay करने से simple tendinitis frozen shoulder बन सकता है, और partial tear complete tear बन सकता है। जितना जल्दी treatment, उतना जल्दी और complete recovery।
